What is D-Dimer test?

During coagulation sequence of reactions occurs in the body in response to variety of external and/or internal stimuli. The enzymatic cascade reaction terminates in the conversion of fibrinogen to fibrin, by the enzyme thrombin.

The fibrin gel is then converted to a stable fibrin clot by thrombin activated Factor XIII. Finally, the fibrin network is dissolved by the enzyme plasmin to generate cross-linked fibrin degradation products (XL-FDP). D-Dimmer comprising of two Dfragments cross linked together is the smallest plasmin resistant molecular unit present with XL-FDP.

What is the significance of D-Dimer test?

Detection of D-dimmer is invaluable as a diagnostic marker for thrombotic condition such as DIC, DVT and PE.

D-dimmer levels can also be used to monitor thrombolytic therapy with tPA and with streptokinase, thrombotic complications in pregnancy, acute myocardial infarction, sickle cell crisis, severe septic infections, liver disease, DIC accompanying snake bite and prognosis and response to therapy in cancer.

What is the significance of D-Dimer test in Covid 19 or Corona virus infection?

Covid-19 is a deadly respiratory disease caused by the noval corona virus. This viral disease spreads through the respiratory droplets releasing from the nose and mouth of an infected person.

D-dimer assays are commonly used in clinical practice to exclude a diagnosis of deep vein thrombosis or pulmonary embolism. Elevated D-dimer level indicates increased risk of abnormal blood clotting. Elevated levels of D-dimer also found to be correlated with higher mortality rate of community-acquired pneumonia.

Coagulation function parameters such as prothrombin time, fibrinogen, fibrin degradation products, and D-dimer levels often found elevated in patients with severe COVID-19 and also the severity of COVID-19 might be associated with coagulation dysfunction.

How can the D-dimer test be useful in Covid-19?

D-Dimer test is used for screening, diagnosis, prognosis and severity assessment of corona virus infection along with other tests such as CBC, CRP, Interleukin-6, Procalcitonin, LDH and serum ferritin. D-Dimer test also help to evaluate the need for anticoagulation treatment in covid 19 patients.
